First Appointment – all Services Please fill out Your First Appointment Download Form or arrive 10 minutes early for any Face to Face Appointment and you can complete a hard copy at the clinic.
Should you wish to have a Telehealth Session you can still print Your First Appointment Download Form and email it if you wish or you can just have the consultation & Dr Phillips will go through some of the questions with you during your session, depending on your health challenge.
